About This Notice
Federal law (HIPAA, the Health Insurance Portability and Accountability Act) protects the privacy of your child's health information and gives families specific rights over it. This page summarizes how Dr. Indira Vemuri MD uses and protects that information.

How We May Use and Disclose Health Information
Treatment, payment, and health care operations
We use your child's health information to provide care, to coordinate with other providers involved in that care (such as specialists, pharmacies, and hospitals), to bill and collect payment from you or your health plan, and to run the practice safely and effectively.
Other permitted uses
The law permits or requires certain disclosures without your authorization, including public health reporting (such as immunization registries), reporting suspected abuse or neglect, health oversight activities, and disclosures required by law or court order.
Uses that require your written authorization
Most uses beyond those above, including marketing and any sale of health information, require your written authorization, which you may revoke at any time. This practice does not sell patient information.
Your Rights Regarding Your Child's Health Information
- Inspect and receive a copy of your child's medical record
- Request corrections (amendments) to the record
- Receive an accounting of certain disclosures
- Request restrictions on certain uses and disclosures
- Request confidential communications (for example, contact at a specific phone number)
- Receive a paper copy of the full Notice of Privacy Practices on request
A Note About Minors and Adolescents
Parents and guardians generally exercise these rights for their minor children. California law gives adolescents independent privacy rights for certain types of care; where state law applies, the practice follows it.
Complaints
If you believe your child's privacy rights have been violated, you may file a complaint with the practice by phone or mail, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will never be penalized for filing a complaint.
Changes to This Notice
The practice may update its privacy practices from time to time. The current notice is always available at the office, and this summary page will be revised to match.
